POLICE RECORD CHECKS:
Before volunteering, everyone (age 18+) must complete and submit to CMHA office the following:
Vulnerable Sector Check - valid three years.
Caledon Offence Declaration - valid one year
Vulnerable Sector Check:
New on-line process.
All team officials, bench staff and volunteers must complete a Vulnerable Sectors Check every 3 years.
Obtain a letter from the CMHA office indicating that you are a volunteer for the organization.
Go to:
Ontario Provincial Police - Police Record Checks and follow the instructions to complete a
Vulnerable Sector Check.
Forward the completed Vulnerable Sector Check to the CMHA office once complete.
Caledon Offence Declaration:
Every year volunteers must submit an 
CALEDON OFFENCE DEC to the CMHA office
- REP volunteers submit to your Rep Convenor and they will submit to the CMHA office .
- House League volunteers please submit to the division convenor and they will submit to the CMHA office
This must be completed before you can be roster to a team or participant in any CMHA activities.
RESIDENTIAL TRANSFER - New to Caledon Minor Hockey (moved into the community):
To register with Caledon Minor Hockey we require access to the Hockey Canada player profile. Please complete the appropriate Player Transfer Form and scan & email to the hockey office at
[email protected] or fax to 905-584-4316, along with the "required supporting documents" to begin the approval process. You will be contacted via email to register once the transfer has been approved.
If your previous centre was an OMHA centre please complete the:
OMHA_to_OMHA_Residential_Transfer with the
15-16_OMHA_Sample_School_Reg_Confirm_Let.
If your previous centre was NOT OMHA, but an OHF member partner, please complete the following and email to the OMHA office

OHF_Player_Transfer with the
OHF_School_Confirm_Letter_15-16.
